TITLE: PUBLIC HEALTH NURSE II

HIRING RANGE: $40,034-$42,035 annually (Exempt - Not eligible for overtime)

STATUS: Full Time

PERSONS WHO ARE BILINGUAL AND ARE WILLING TO PROVIDE TRANSLATION

SERVICES WILL RECEIVE AN EXTRA 5% INCENTIVE PAY.

ESSENTIAL DUTIES: (NOTE: The duties listed below are not all of the duties that may be assigned to the position but are considered as essential to the nature of the position).

· Conducts a needs assessment interview with patients in the Gaston County Health Department and/or in their residence as a pre-clinic visit.

· Evaluates personal history; provides specific education/health treatment plan for patient.

· Provides case management services to the Health Department patients.

· Conducts visits to local day care facilities to assure compliance with communicable disease regulations; provides written and statistical documentation for state and federal regulatory agencies.

· Provides medical care as needed.

· Provides home visits for assessments and/or follow-up when indicated.

MINIMUM QUALIFICATIONS:

Graduation from an accredited college or university with a Bachelor's degree in Nursing which includes a public health nursing rotation and 1 year experience; OR graduation from an accredited school of nursing and two years of professional nursing including one year in public health; or an equivalent combination of education and experience. Must be licensed by the state of North Carolina as an RN. A valid driver’s license is required. Preference will be given to Spanish speaking applicants. The candidates selected must undergo a criminal background check and take and pass a pre-employment drug screening test prior to employment.
